[Drug classification] The currently applied lipid regulating drugs are mainly divided into the following categories: 1. Statins such as lovastatin, simvastatin and the like. 2. Clofibrate and phenoxyacetic acids such as clofibrate, fenofibrate and gemfibrozil. 3. Niacin and its derivatives such as niacin, inositol niacin and so on. 4. Cholic acid chelating agents such as cholestyramine. [Mechanism] Atherosclerosis is mainly caused by disorder of lipid metabolism and decreased fibrinolytic activity. Therefore, adjusting blood lipid metabolism can prevent atherosclerosis, and the mechanism of action of blood lipid regulating drugs has the following aspects. 1. The role of statins is to inhibit the in vivo production of cholesterol. 2. The blood lipid-lowering effect of clofibrate is to increase the lipopolysaccharide-rich protein and catabolism, increase the catabolism of triglyceride-rich protein, inhibit the synthesis of cholesterol and triglycerides, and increase sterols. Excretion. 3. The hypolipidemic effect of niacin and its derivatives may reduce the production of very low-density lipoprotein by inhibiting the fat dissolution of adipose tissue and reducing the release of free fatty acids into the liver. 4. Cholic acid chelating agents can affect the absorption of cholesterol and bile acids. [Clinical application] 1. Statins lower blood cholesterol and low-density lipoprotein levels by 15% to 60%, and also have a slightly weaker effect on blood-lowering triglycerides and elevated plasma high-density lipoprotein. Lipid effects are dose related. 2. Chloroprene decreases blood triglycerides more strongly than cholesterol. 3. Nicotinic lowering of triglycerides is stronger than lowering blood LDL, and also increases blood HDL. 4. Cholic acid chelators reduce blood LDL levels and increase high density protein levels.
